Cresemba (isavuconazonium sulfate) is a prescription antifungal medication used to treat serious fungal infections, including invasive aspergillosis and invasive mucormycosis. It is a prodrug that is rapidly converted in the body to isavuconazole, which inhibits the synthesis of fungal cell membranes, leading to fungal cell death.
Benefits
- Effective against life-threatening fungal infections such as aspergillosis and mucormycosis
- Broad-spectrum activity against a wide range of yeasts and molds
- Available in both oral and intravenous formulations for flexible treatment
- Generally better tolerated than older antifungal agents like amphotericin B
- Suitable for long-term treatment when necessary
Dosage
- Typical loading dose: 372 mg (equivalent to 200 mg isavuconazole) every 8 hours for 6 doses
- Maintenance dose: 372 mg once daily starting 12–24 hours after the last loading dose
- Can be taken orally (capsules) or intravenously — interchangeable formulations
- Capsules should be swallowed whole, with or without food
- Duration of therapy depends on the type and severity of infection — follow medical guidance
Warnings and Precautions
- May cause liver toxicity — monitor liver function tests regularly
- Use with caution in patients with known cardiac issues; Cresemba may shorten the QT interval
- Not recommended for use with strong CYP3A4 inducers or inhibitors due to interaction risk
- Use in pregnancy only if clearly needed; may cause fetal harm
- Safety in breastfeeding is not established — consult a healthcare provider
- Not indicated for fungal infections that are less severe or self-limiting

- Take at the same time each day for consistent blood levels
